Our office is located at 4650 President George Bush Hwy, Suite 210, in Sachse, TX. The building is situated along President George Bush Turnpike (PGBT/Highway 190) and is easily accessible from multiple directions.

From Dallas / West

Take President George Bush Turnpike (Highway 190) east. Our office is located on the south side of PGBT in Sachse. Look for 4650 President George Bush Hwy and proceed to Suite 210 on the second floor.

From Plano / North

Head south on US-75 or the Dallas North Tollway and merge onto President George Bush Turnpike (Highway 190) heading east. Continue on PGBT through Richardson and Garland into Sachse. Our office is on the south side of the highway.

From Murphy / Wylie / East

Take Highway 78 or FM 544 west toward Sachse. Connect to President George Bush Turnpike (Highway 190) heading west. Our office is located at 4650 President George Bush Hwy, Suite 210.

From Garland / Rowlett / South

Head north on Highway 66 or Highway 78 and merge onto President George Bush Turnpike (Highway 190) heading east. Our office is in the 4650 building on the south side of PGBT in Sachse.

Parking

Complimentary parking is available in the lot surrounding the building at 4650 President George Bush Hwy. There is ample surface parking near the building entrance. Accessible parking spaces are available near the main entrance for patients who need them.

What to Bring to Your First Visit

To help your first appointment go smoothly, please bring the following items:

• Photo identification — A valid government-issued photo ID such as a driver's license or passport.
• Insurance card — Your current health insurance card (front and back). If your insurance has changed recently, please bring your most up-to-date card.
• Referral letter — If your insurance plan requires a referral from your primary care physician, please bring the referral authorization or ensure it has been sent to our office before your visit.
• Medication list — A complete list of all current medications, including dosages, as well as any over-the-counter supplements or vitamins you take regularly.
• Medical records — Any relevant prior medical records, lab results, imaging reports, or procedure reports from other providers that may be pertinent to your visit. If these have been sent electronically, please confirm with our office that we have received them.
• List of questions or symptoms — Writing down your symptoms, their duration, and any questions you have for Dr. John will help ensure that your concerns are fully addressed during the appointment.
• Copay or payment — Copays and any patient-responsibility balances are collected at the time of service.

Please arrive 15 to 20 minutes before your scheduled appointment time to allow for check-in and paperwork. If you have completed any patient intake forms in advance, please bring them with you or submit them electronically as instructed by our office staff.
